We are participating providers with Medicare and accept assignment of all Medicare claims.  This means that Medicare pays 80% of the allowed charge.  The patient is directly responsible for the remaining 20% as well as the annual deductible.  If you have a secondary insurance for the remaining 20% we will file that claim as a courtesy.

We participate in most HMO, PPO and managed care programs.  It is your responsibility to obtain any referrals from your primary care physician or your insurance company if these are required.  All copayments are due at the time of every office visit.  We also accept Visa and MasterCard.

We will be glad to file your insurance claims for you, however you are ultimately responsible for full payment of the account balance.  All deductibles and 20% portions are expected at the time of service.  A payment schedule may be arranged for larger balances such as those for surgical procedures.
