Preparing For Your Visit & What To Expect

At Retina Associates, P.A., we want to ensure every patient knows what to expect when they arrive for their appointment. When you arrive at one of our offices for your appointment, you will be greeted by our receptionist who will help check you in and make sure we have all of your current contact, health and insurance information. 

*Patients are being seen on an emergency basis, please understand that your appointment may take longer than normal. We want to ensure all patients receive the same care and treatment.

We ask that you allow at least two hours for your scheduled appointment due to various tests and procedures that may occur. Below is a list of items you will need to bring with you.

  • Completed new patient forms
  • A list of all the medications you are currently taking
  • Eye drops you are currently using
  • Your medical insurance card
  • A photo identification card
